What is VR therapy?

Virtual reality therapy, often referred to as VR exposure therapy, uses VR headsets to immerse patients in computer-generated yet deceptively realistic environments. Unlike traditional exposure therapy in the real world, your therapist maintains complete control over the intensity, duration, and pace of the situation at all times. Height, crowds, confined spaces, or specific triggers can be gradually increased, paused at any time, and precisely tailored to your individual stress tolerance.

This precise controllability makes VR therapy a particularly effective and, at the same time, gentle component of behavioral therapy. VR Therapy for Anxiety Disorders and Phobias

Virtual reality therapy is one of the most extensively studied applications for anxiety disorders and specific phobias. It allows for exposure in situations where real-world practice scenarios are often difficult, time-consuming, or impossible to recreate—such as at great heights, on an airplane, or in a crowded place.

Examples: Fear of Heights and Fear of Flying

A patient with a severe fear of heights first steps onto a low observation deck in the virtual environment, and later onto a glass bridge or into an open elevator at a great height. Her therapist gradually increases the height while she practices relaxation and coping techniques at the same time. In the case of a fear of flying, a cabin—including takeoff, cruise phase, and turbulence—can be simulated in the same way, completely controlled and without any real risk.

Example: Social Anxiety Disorder and Agoraphobia

People with social anxiety practice, for example, giving a presentation in front of an audience or having a conversation in a group—situations that are often avoided in real life. For agoraphobia or panic disorder, virtual scenarios such as a crowded supermarket, a subway ride, or an elevator can be worked through step by step until avoidance behavior noticeably decreases.

Scientific Evidence

The effectiveness of VR exposure therapy for anxiety disorders and specific phobias is considered well-established. The S3 guideline on the treatment of anxiety disorders issued by the German Society for Psychiatry and Psychotherapy, Psychosomatics, and Neurology (DGPPN) explicitly recommends virtual reality as a therapeutic tool for specific phobias, integrated into guideline-based behavioral therapy.

A widely cited meta-analysis of randomized controlled trials—comprising 30 studies and over 1,000 participants—found a large effect size for VR exposure therapy compared to waiting-list conditions, as well as a medium-to-large effect size compared to active control conditions. In a direct comparison with traditional in-vivo exposure, no statistically significant differences in efficacy were found; virtual exposure thus proved to be an equally effective alternative, particularly in situations where a real-life exposure exercise is difficult to implement.

VR Therapy for Obsessive-Compulsive Disorder

In cases of compulsive checking—such as repeatedly checking the stove, door locks, or electrical appliances—a virtual environment can be used to simulate a living situation that must be left without performing the habitual checking behavior. This breaks the cycle of compulsion in a safe setting and helps establish new, anxiety-free behavior.

Scientific Evidence

Exposure with response prevention is considered the first-line treatment for obsessive-compulsive disorder (OCD) according to the S3 guidelines. The use of virtual reality (VR) to implement this approach is a relatively new and rapidly growing field compared to research on phobias: A randomized controlled trial conducted by the University Medical Center Hamburg-Eppendorf involving 80 patients with washing or checking compulsions specifically examined the safety and tolerability of VR-assisted exposure with response prevention and yielded positive results: side effects were rare, and the procedure proved to be safe and well tolerated.

The professional literature still classifies the body of research on VR applications for obsessive-compulsive disorder as more recent and still in its early stages compared to that for specific phobias.
